Requirements
- A current passport valid for at least six months past the expected time of arrival in the U.S and any previous passports showing travel outside Sri Lanka
- Original DS-160 Non Immigrant Visa Application (NIV) form (Confirmation page of each applicant).
- The photograph must be 2″x2″ and have a white background. Non-compliant photographs will be rejected and the applicant will have to reapply
- Original birth certificate
- Bank draft for certified funds in the amount of US$160 or Rs.21,600 drawn in favour of “U.S. Embassy Colombo” for categories under B,C,D,F,J & M.
- Marriage certificate
- Children’s birth certificates
- Personal and business bank statements and bank books
- Deeds to any property owned
- Proof of employment (pay slip, EPF/ETF slips, tax returns)
- Proof of education
- Evidence of sponsor or host’s legal status in the U.S. (copy of passport, green card or visa);
- Financial statements of businesses owned and tax returns;
- Travel Itinerary with name of travel agent.
- Passport valid for travel to the United States – Your passport must be valid for at least six months beyond your period of stay in the United States

- Application fee payment receipt, if you are required to pay before your interview.

- Above application fee will be non-refundable
- Please write your full name and passport number on the back of the bank draft. Applicants will not be allowed in the Embassy without their bank draft
